Shogun Licence Application
Please enclose any previous licences held, these will be returned after transferring all relative data. All Licence fees are non-refundable. In the event of cessation of training, relocation, or moves to any other Organisation, there will be no refund of any description. Payment denotes full acceptance of this rule. Please keep all Record Books safe, as duplicates will require a £12 replacement fee. The General Data Protection Regulation (GDPR) replaces the Data Protection Act 1998 on the 25th May 2018. The Shögun Licencing Database will operate GDPR Compliant. Signatures denotes acceptance of your addition to our Licence Database. We will not share your information. All Licences must run consecutively, in order to maintain Grade and Insurance Premium. If a Student ceases training for any reason, their Licence also becomes invalid, to prevent claims for injuries created or sustained elsewhere.
